I authorize Ortiz Towing & Recovery to tow, transport, recover, move, load, unload, or otherwise handle the vehicle identified above. I represent that I am the owner of the vehicle or am authorized by the owner to request this service.
I acknowledge the opportunity to identify existing damage before service and understand that towing and recovery may involve ordinary risks. To the extent permitted by law, Ortiz Towing & Recovery is not responsible for pre-existing damage, hidden defects, defective or unsafe vehicle components, loose personal property, road or site conditions, or damage that could not reasonably be avoided while performing the requested service with reasonable care. This form does not waive liability that cannot legally be waived.
I acknowledge that the photos and condition notes above are intended to document the vehicle's visible condition before towing or recovery. I agree to pay the lawful towing/recovery charges shown or otherwise properly due.
